Patient Advocates
are committed to supporting the needs of our patients. We employ people in four areas to personally assist our patients:
Patient Relations Representatives are available Monday through Friday to assist patients with special needs. They also respond to patient requests and complaints.
Patient Financial Representatives are available Monday through Friday on the first floor of the administrative annex building. Patient Financial Representatives assist patients who may be eligible for public funding of their medical care. Patients or their designated representative may meet with staff either while they are inpatients or after they are discharged. Follow-up meetings can be arranged in private offices located at the hospital.
Social Workers meet with patients and family to understand the patient’s psychosocial strengths and needs. Social workers are part of the care team and act as an additional patient advocate extending protection (in cases of crisis), support and education to the patient and family. Social workers are available to work with physicians, nurses and discharge planning. They make financial, transportation, custodial and community assistance referrals.
Discharge Planners review the medical record of every patient and work with the patient to arrange for the medical care that the patient will need after the patient leaves the hospital. This may include home health, follow-up physician visits, skilled nursing, etc.
