What do I do if I need my medications refilled?

Refills should be requested from your pharmacy w/at least 7 days in advance.  The majority of our physicians are not here every day, this will give us time to refill your prescription in a timely manner in addition to correcting any problems that may arise, such as the medicine not being currently covered by your insurance or too much time (>3months) has elapsed since your last office visit.

What do I do if I need copies of my medical records?

Please come in to sign a record release in order to give us permission to copy your medical records and to clarify if these records are for you or need to be sent to another physician.

How will I know how much the clinic visit will cost?

We accept all insurances except HMOs or special medical plans. However, if there is no coverage we will be happy to offer you our Sliding Fee program. This program bases the visit cost on your income and the number of dependents in your household. Should the patient not qualify for the Sliding Fee program, the cost of the visit will be anywhere from $150 or more depending on the services for that date of service

What types of payment methods do you accept?

We accept all insurances except HMOs, special medical programs and out-of-county CMSP. Children’s Specialists Clinic is the only outpatient facility in our group that accepts CCS. Cash, check and all major Credit Cards are accepted for payments.

All of your medicines prescribed by every doctor you see must be brought in to every appointment. Some specialists will also require that you bring your latest CT, Ultrasound or X-Ray films.

What information will the clinic need in order to register me?

We will need your insurance card and a valid picture ID – you will also be required to provide us your up-to-date contact information and a brief health history questionnaire. For your convenience, the questionnaire can be picked up prior to your appointment to be filled out in the comfort of your home.
